Consumer Rights
You have the right:
- To be treated with dignity and respect.
- To have their records and conversations with people who give care kept private.
- To take part in decisions about their care.
- To receive services without regard to race, color, religion, sex, sexual orientation, age or ethnic background.
- To see their records and talk about them with their provider.
- To file a complaint or file a grievance about the services they receive without any type of punishment.
- Consumer Responsibilities
Your responsibilities as a Consumer are to:.
- Treat the service providers with dignity and respect.
- Give the Peer Specialist/Provider the information needed to provide service.
- Help the Peer Specialist/Provider understand your problems.
- Work with the Peer Specialist/Provider to develop an individualized recovery plan.
- Ask the Peer Specialist/Provider questions so that you understand the service plan.
- Follow the individualized service plans you have developed and agreed to with the Peer Specialist/Provider.
- Keep appointments or cancel and reschedule them when you are not able to keep them.
- Notify the Peer Specialist/Provider and/or your Member and Provider Service Representative or Service Manager if you move or decide to stop services for any reason.
